Who can use FORM-A
You must be 18 or older. You are responsible for the accuracy of the information you give us.
What FORM-A is
FORM-A provides AI-generated visual projections and general fitness and nutrition guidance. It is not a medical service and does not diagnose, treat or prevent any condition.
Projections
Projection images are AI-generated illustrations based on your photos and estimated numbers. They are not predictions, promises or guarantees of results. You may not use projections of other people or upload photos of anyone but yourself.
Subscriptions
FORM-A Pro renews automatically until cancelled. You can cancel any time in Settings; access continues until the end of the paid period. Free trials convert to paid plans unless cancelled before the trial ends.
Acceptable use
Do not misuse the service, attempt to access other users' data, or use generated images to deceive or harass anyone.
Liability
To the extent allowed by law, FORM-A is provided “as is” and we are not liable for injury or loss arising from following a plan. Always train within your limits.
